Application to Use Library Services for Alumni of the Northwestern University

Only alumni of the Feinberg School of Medicine (FSM) and the Dental and Nursing Schools at Northwestern University are eligible to register for library services at the Galter Health Sciences Library. All other alumni should contact the library of the school from which they graduated to find out about services offered to alumni of other schools at Northwestern University.

If you have already received a library barcode, you do not need to fill out this form. You may proceed to use the barcode to register for library services. Please call the Reference Desk at (312) 503-8109 if you have any questions.
